Aurora	Community Home Assistance To Seniors (CHATS) is a not-for-profit organization based on a passion for caring and community, and specializes in providing support and care to seniors and their caregivers within York Region.
Leamington	The Leamington District Memorial Hospital's mission is "dedicated to improving the health status of the people we serve. With compasison and in partnership with others we will strive to provide quailty, continuity of service and excellence in health care."
Orangeville	Community Care Access Centre Wellington-Dufferin is a facilitator of health services and health information across the regions of Wellington and Dufferin. They are working in collaboration with Headwaters Health Care Centre.
Paris	PrimaCare Health is a network of family physicians local to Paris (ON) and surrounding communities, who work together with other health care profesisonals and the Willett Hospital to give you improved access to medical help and information when you need it.

Thorold	VON Niagara is a charitable, not for profit health care organization that has provided bursing and home support programs and services in the Niagara peninsula since 1919.
Tillsonburg	The Tillsonburg Community Center is a dynamic and dedicated team of qualified staff, volunteers and community partners who are committed to strengthening individuals, families and the community by offering lifelong learning, leisure and recreational experiences.
Wallaceburg	VHA Home Healthcare is a not-for-profit home health and support agency with over 80 years of experience in community healthcare.
Woodstock	VON Oxford is an accredited not for profit charitable organizaion with over 91 years of experience in delivery of community health and social services.
