Appendix 10.1: Information Management: Clinforma Privacy Information & Regulations


Accessibility

All personal health information gathered at CHAP pharmacy sessions and sent to Clinforma via either the fax-to-database system or the web-based data system is accessible only to Clinforma staff who have a designated need to access this information for purposes of project data management only.

Clinforma staff:

  • Have over 10 years experience in handling patient data in this series of projects and over 15 years in many other clinical studies;
  • Are certified by McMaster University Research Ethics Board for handling patient health records;
  • Are bound by a written, signed Confidentiality Agreement with their employer.
Security

Patient information that is faxed to Clinforma is held on secure, password-protected servers which are themselves securely housed in an always-locked, ventilated, dedicated server room within Clinforma's 10th-floor downtown Hamilton office tower location. Clinforma premises, as a whole, are kept locked at all times. During off hours, premises are further protected by both a monitored and registered security alarm system and the physical denial of access by elevator to the entire floor without electronic ID card.
